New Clinical Trial Finds CBD Reduced Chronic Nerve Pain After Spinal Cord Injury

A newly published clinical trial is giving us another reason to pay attention to CBD research, especially when it comes to chronic nerve pain.

Researchers in Australia found that high-dose CBD significantly reduced neuropathic pain in adults living with spinal cord injuries compared with placebo.

And this wasn’t an observational study or simply a collection of patient experiences.

It was a randomized, double-blind, placebo-controlled clinical trial.

The study was published in eClinicalMedicine in May 2026.

What Is Neuropathic Pain?

Neuropathic pain is pain caused by damage or dysfunction within the nervous system itself.

People often describe it as:

  • burning
  • shooting
  • electric
  • stabbing
  • tingling
  • pins and needles
  • painful sensitivity to touch

It can behave very differently from the pain someone might experience from a sore muscle or an inflamed joint.

And unfortunately, chronic neuropathic pain is extremely common after spinal cord injury.

The researchers behind this study noted that more than two-thirds of people with spinal cord injuries experience chronic neuropathic pain, and for many, it can become one of the most debilitating long-term consequences of the injury.

How Do Spinal Cord Injuries Happen?

Spinal cord injuries can happen in more ways than people may realize.

In the United States, vehicle crashes are one of the leading causes, followed by falls and acts of violence.

Sports and recreational accidents also account for a portion of spinal cord injuries.

That can include things like diving accidents and high-impact sports, including football and other contact sports.

Once the spinal cord is damaged, the nerves responsible for carrying messages between the brain and body may also be damaged.

For some people, that can lead to chronic nerve pain that continues months or even years after the original injury.

What Did Researchers Study?

Researchers recruited 40 adults with spinal cord injuries who had been experiencing neuropathic pain for at least three months.

Thirty-eight participants were ultimately included in the primary analysis.

The study used what’s known as a crossover design.

That meant participants received both CBD and placebo at different points in the trial, allowing researchers to compare how the same individuals responded under each condition.

Participants completed:

  • six weeks taking CBD
  • six weeks taking placebo

The two treatment periods were separated by a four-week break.

Neither participants nor the researchers working directly with them knew which treatment they were receiving during each phase.

How Much CBD Did They Use?

This is an important part of the study.

Researchers used pure oral CBD, without THC.

The dose was gradually increased over the first two weeks:

  • 200 mg per day
  • 400 mg per day
  • 600 mg per day
  • eventually reaching 800 mg per day

Participants remained at the highest dose through the remainder of the treatment period.

So this was definitely a high-dose CBD study.

Researchers were specifically interested in whether doses much higher than those used in several previous pain trials might produce different results.

What Did They Find?

CBD significantly reduced participants’ self-reported pain intensity compared with placebo.

By the final week of treatment, participants experienced an average pain reduction of approximately 14% while taking CBD, compared with approximately 6.5% while taking placebo.

The researchers also looked at how many participants experienced a more substantial reduction in pain.

Nearly 38% of participants experienced at least a 30% reduction in pain while taking CBD, compared with about 11% while taking placebo.

That difference was statistically significant.

Researchers described the overall improvement as modest, but meaningful enough to support further study of high-dose CBD for chronic neuropathic pain.

This Was CBD Without THC

One of the reasons this study caught our attention is that researchers were specifically studying CBD itself.

The treatment contained highly purified CBD and did not rely on THC to produce the effect seen in the trial.

That matters because cannabinoid pain research often involves products containing both CBD and THC, which can make it difficult to determine which cannabinoid is responsible for the result.

In this study, researchers were able to look directly at the effects of CBD.

Why Might CBD Affect Nerve Pain?

Scientists are still working to understand all of the ways CBD interacts with the nervous system.

CBD does not simply bind to one receptor and create one effect.

Instead, researchers have found that it influences a number of signaling pathways involved in:

  • pain perception
  • inflammation
  • nerve signaling
  • stress response
  • serotonin signaling
  • the body’s endocannabinoid system

The endocannabinoid system plays an important role in helping regulate communication throughout the nervous system.

Because neuropathic pain originates within damaged or altered nerve pathways, researchers have become increasingly interested in whether cannabinoids like CBD may influence the way those pain signals are processed.

Animal and laboratory studies have suggested this possibility for years.

Human clinical trials like this one are where researchers begin determining whether those effects translate into meaningful changes in actual patients.

What About Other Symptoms?

Researchers also evaluated a number of secondary outcomes, including:

  • different characteristics of neuropathic pain
  • anxiety
  • depression
  • stress
  • sleep
  • the impact of pain on quality of life

Most of these secondary measures did not significantly change compared with placebo.

The clearest finding from the trial was the reduction in pain intensity itself.

And that’s an important distinction.

Good research isn’t about finding a positive result everywhere researchers look.

It’s about identifying where a treatment did and did not make a measurable difference.

Was the CBD Well Tolerated?

Researchers reported that CBD was generally well tolerated.

Adverse events occurred somewhat more frequently during CBD treatment than placebo, but nearly all were considered minor.

That is particularly notable considering the dose reached as high as 800 mg per day.

Why This Study Matters

CBD and pain may be one of the most talked-about areas of cannabinoid use.

People have been using CBD for discomfort and chronic pain for years.

But clinical research hasn’t always produced consistent results.

Some previous randomized trials using much lower doses of CBD did not find significant improvements in neuropathic pain.

This study took a different approach by testing substantially higher doses.

And this time, researchers found a statistically significant difference between CBD and placebo.

That doesn’t necessarily mean that more CBD is always better.

But it does raise an incredibly interesting question about whether dose may be one of the reasons CBD pain studies have produced such different results.

The researchers themselves noted that high-dose CBD deserves further investigation in larger clinical trials.

Another Piece of the CBD Puzzle

This is what makes cannabinoid research so fascinating.

We’re no longer simply asking:

Does CBD work?

Researchers are beginning to ask much more specific questions.

What type of pain?

What dose?

How long should it be used?

Why do some people respond more strongly than others?

Does genetics affect response?

Does the underlying cause of the pain matter?

Those are much more useful questions.

And interestingly, this study found early evidence suggesting that some people may respond considerably better to CBD than others.

Researchers even examined genetic differences related to the body’s endocannabinoid system as one possible explanation.

That’s something we will be very interested to see explored further.

The Research Keeps Moving Forward

For more than a decade, we’ve watched the science surrounding CBD evolve.

Some studies produce exciting results.

Others don’t.

But each well-designed human clinical trial gives researchers another piece of information about how cannabinoids actually interact with the human body.

This study adds an important one.

In a randomized, double-blind, placebo-controlled human trial, high-dose CBD significantly reduced chronic neuropathic pain associated with spinal cord injury.

The improvement wasn’t enormous.

But it was measurable.

And for people living with chronic nerve pain, even modest improvements can matter.
